download the forms from the state website. if you do it wrong and the forms are rejected, you lose your filing fee and have to pay again when you resubmit
You can go to your local Office Max and purchase the ” Do It Yourself Incorporation Kit” for next to nothing or you can contact the local agency and I believe it cost $75.
